I'd say the later. Kind of Game of Thrones. You can do it without being graphic, again see GRR Martin's novels vs. the television series.
Was it dangerous back then? Yes. But, I'm not sure it needed to be used as a plot device and a means to further character?
I don't know, it bothered some more than others. Most of the fans have no issues with it. It's just why I backed away.
